If your student is attending here this year your process for re-enrolling will be much shorter than first time enrollment and can be handled online. If you have any issues accessing online services or questions about the process, please just let us know. We are here to help!
If your student is needing a physical health exam by a medical professional (this includes all new students, kindergarteners and seventh graders, you will need a printed paper form for your medical professional to complete. The form is available here for downloading so you can print before you go for your child's appointment. You can also get copies of this form from our office.
This form needs to be completed and signed by a medical professional if your student will need medication during school hours, whether we administer the meds or the student does. The form is available here for you to download and print. You can get copies of this from our office as well.
